Currently Branch of Life Midwifery, LLC is unable to accept insurance. However, I will make every effort to help you with a superbill after your birth so you can try to submit for your own reimbursement if your insurance covers CPM's. This is the patient's responsibility.
Labs, ultrasounds, and other professional consultations as indicated may be covered by insurance.
Branch of Life Midwifery, LLC has a VERY limited availability and due to very restrictive Arizona laws will only be accepting clients that have had a prior successful vaginal birth. VBAC's may be accepted if client has had a successful vaginal birth after the cesarean. Branch of Life Midwifery, LLC will not be currently attending breech or twin births at home. Please check out the individual midwifery practice guideline document for more information.
